Common use

Sildamax is primarily prescribed for the management of erectile dysfunction, a condition characterized by the consistent inability to attain or maintain an erection sufficient for satisfactory sexual performance. It is used on an as-needed basis, taken prior to anticipated sexual activity. It is not an aphrodisiac and requires sexual stimulation to be effective. Its use is confined to adult males and is not indicated for use by women, children, or adolescents.

Dosage and direction

The recommended starting dose for most patients is 50mg, taken approximately one hour before sexual activity. Based on efficacy and tolerability, the dose may be increased to a maximum recommended dose of 100mg or decreased to 25mg. The tablet should be swallowed whole with a glass of water. Sildamax can be taken with or without food; however, a high-fat meal may delay the onset of action. The medication should not be taken more than once per day.

Precautions

Prior to taking Sildamax, a thorough medical evaluation is essential to diagnose erectile dysfunction and identify any underlying health conditions. Patients with cardiovascular disease should be assessed for cardiac safety before initiating any treatment for ED, as sexual activity carries a potential cardiac risk. Use with caution in patients with anatomical deformation of the penis, bleeding disorders, or active peptic ulceration. This medication can cause hypotension; caution is advised in patients taking antihypertensive drugs.

Contraindications

Sildamax is contraindicated in patients with a known hypersensitivity to Sildenafil Citrate or any of the tablet’s excipients. Its concurrent use with any form of organic nitrate therapy (e.g., nitroglycerin, isosorbide mononitrate or dinitrate) or guanylate cyclase stimulators (such as riociguat) is absolutely contraindicated due to the risk of a severe, potentially fatal drop in blood pressure. It is also contraindicated in men for whom sexual activity is inadvisable due to underlying cardiovascular status.

Possible side effects

Like all medicines, Sildamax can cause side effects, although not everybody gets them. Very common (may affect more than 1 in 10 people) and common (may affect up to 1 in 10 people) side effects include headache, flushing, and indigestion. Uncommon side effects (may affect up to 1 in 100 people) include visual disturbances (such as altered colour vision, blurred vision), dizziness, nasal congestion, and rash. Rare but serious side effects include priapism (a prolonged and painful erection lasting more than 4 hours), sudden decrease or loss of vision, and sudden decrease or loss of hearing. Medical attention should be sought immediately if any serious side effects occur.

Drug interaction

Sildamax can potentiate the hypotensive effects of nitrates and alpha-blockers (e.g., doxazosin), and concomitant use is contraindicated or requires careful medical supervision. It is metabolized primarily by the CYP3A4 enzyme system; therefore, potent inhibitors of CYP3A4 (e.g., ketoconazole, ritonavir, erythromycin) can increase Sildenafil plasma concentrations. Inducers of CYP3A4 (e.g., rifampicin, phenobarbital, carbamazepine) may decrease Sildenafil concentrations. Concomitant use with other PDE5 inhibitors is not recommended.

Missed dose

Sildamax is not intended for chronic daily dosing and is taken on an as-needed basis. Therefore, the concept of a “missed dose” does not apply. The tablet should simply be taken when needed, approximately one hour before planned sexual activity, adhering to the maximum frequency of once per day.

Overdose

In cases of overdose exceeding the maximum recommended dose, the side effects are expected to be an increase in the severity of the known dose-related adverse events. Standard supportive measures should be employed as required. Given that Sildenafil is highly protein-bound, it is not expected to be dialyzable. Management of a severe hypotensive event may require fluid administration and other appropriate supportive measures, noting that organic nitrates are contraindicated.

Storage

Store in the original package below 30°C (86°F) to protect from moisture and light. Keep the medication out of the sight and reach of children. Do not use Sildamax after the expiry date which is stated on the blister and carton after “EXP”. The expiry date refers to the last day of that month.
