Common use

Nasonex is primarily indicated for the treatment of seasonal and perennial allergic rhinitis symptoms in adults and children ages 2 years and older. It is also approved for the prophylaxis of seasonal allergic rhinitis symptoms when initiated 2-4 weeks prior to anticipated pollen exposure. Additionally, it is used as an adjunctive treatment for nasal polyps in patients 18 years and older, helping to reduce polyp size and improve nasal airflow.

Dosage and direction

For allergic rhinitis in adults and adolescents 12 years and older: 2 sprays in each nostril once daily (total 200 mcg/day). For children 2-11 years: 1 spray in each nostril once daily (total 100 mcg/day). For nasal polyps in adults: 2 sprays in each nostril twice daily (total 400 mcg/day). Shake well before use. Prime the pump before first use by actuating 10 times until a fine spray appears. If not used for 14 days or longer, reprime with 2 sprays. Blow nose gently before use. Tilt head slightly forward and insert tip into nostril, pointing away from nasal septum. Breathe in gently while spraying.

Precautions

Use with caution in patients with recent nasal septum ulcers, nasal surgery, or nasal trauma. Monitor for signs of nasal candidiasis. Patients on immunosuppressant drugs are more susceptible to infections. Use caution in patients with active or quiescent tuberculosis infections, untreated fungal, bacterial, or viral systemic infections, or ocular herpes simplex. Long-term use may suppress hypothalamic-pituitary-adrenal (HPA) function. Regular examinations of the nasal mucosa are recommended during prolonged treatment.

Contraindications

Hypersensitivity to mometasone furoate or any component of the formulation. Not for use in patients with untreated localized nasal infections. Contraindicated in patients with active or quiescent tuberculous infections of the respiratory tract, untreated fungal, bacterial, or viral systemic infections, or ocular herpes simplex.

Possible side effects

Common side effects (≥2%) include: headache, nasal irritation, pharyngitis, epistaxis (blood-tinged mucus), and nasal burning. Less common effects may include: nasal ulceration, candidiasis, impaired wound healing, increased intraocular pressure, glaucoma, cataracts, and adrenal suppression with prolonged use. Systemic corticosteroid effects may occur with excessive doses.

Drug interaction

No formal drug interaction studies have been conducted with Nasonex. However, caution is advised with concomitant use of strong CYP3A4 inhibitors (ketoconazole, ritonavir) which may increase mometasone systemic exposure. Monitor patients using other nasal corticosteroids concurrently for increased systemic effects.

Missed dose

If a dose is missed, use it as soon as remembered. If it is almost time for the next dose, skip the missed dose and resume regular dosing schedule. Do not double the dose to make up for a missed dose.

Overdose

Acute overdose is unlikely due to low systemic bioavailability. Single doses up to 20 times the recommended dose have been administered without adverse effects. Chronic overdose may lead to systemic corticosteroid effects including hypercorticism and adrenal suppression. Treatment should be symptomatic and supportive.

Storage

Store at 20-25°C (68-77°F); excursions permitted to 15-30°C (59-86°F). Do not freeze. Keep bottle upright. Discard 120 days after opening or when the counter reads “0”, whichever comes first. Keep out of reach of children.
