Common use

Dosage and direction
Dosage is dependent on the chosen formulation and must be strictly adhered to as prescribed. A single dose should be taken at the onset of migraine symptoms. A second dose may be taken if symptoms improve but then return, but no sooner than 2 hours for tablets or nasal spray, and 1 hour for injection. The maximum daily dose should not be exceeded.
- Tablets: The recommended single dose is 25 mg, 50 mg, or 100 mg. The maximum single dose is 100 mg, and the maximum daily dose is 200 mg.
- Nasal Spray: A single 5 mg, 10 mg, or 20 mg dose is administered in one nostril. The maximum daily dose is 40 mg.
- Subcutaneous Injection: A single 6 mg dose is administered. The maximum daily dose is 12 mg, administered in two separate injections at least 1 hour apart.
The medication is for acute use only. The safety of treating more than four headaches in a 30-day period has not been established.
Precautions
Prior to initiating treatment with Imitrex, a thorough cardiovascular evaluation is recommended for patients with risk factors for coronary artery disease. Imitrex may cause chest pain, tightness, pressure, or heaviness; patients experiencing these symptoms should be evaluated by a physician before subsequent use. Use with caution in patients with uncontrolled hypertension, hepatic impairment, or a history of epilepsy or structural brain lesions. Imitrex should not be used within 24 hours of another triptan or ergotamine-type medication. Overuse of acute migraine treatments can lead to medication-overuse headache; patients should monitor their frequency of use.
Contraindications
Imitrex is contraindicated in patients with:
- Ischemic heart disease (e.g., angina pectoris, history of myocardial infarction, documented silent ischemia).
- Coronary artery vasospasm, including Prinzmetal’s angina.

- History of stroke or transient ischemic attack (TIA).

Drug interaction
Concomitant use of Imitrex with the following medications is contraindicated or requires extreme caution due to the risk of prolonged vasospastic reactions:
- Ergotamine-containing medications (e.g., Ergomar, Cafergot) or ergot-type medications (e.g., dihydroergotamine, methysergide).
- Other 5-HT1 agonists (other triptans like rizatriptan, zolmitriptan).
Use with Monoamine Oxidase (MAO) inhibitors is contraindicated. Using Imitrex with Selective Serotonin Reuptake Inhibitors (SSRIs) or Serotonin and Norepinephrine Reuptake Inhibitors (SNRIs) may potentially increase the risk of serotonin syndrome, a rare but serious condition.
Missed dose
Imitrex is not taken on a scheduled basis; it is used only to treat an acute migraine attack as it occurs. Therefore, the concept of a “missed dose” does not apply. The medication should be taken at the onset of symptoms. Do not take a dose to “make up” for not taking one earlier in the day.
Overdose
In cases of overdose, symptoms could be an extension of the drug’s adverse effects. Symptoms may include hypertension, more pronounced cardiovascular events, seizures, and a significant increase in the drug’s common side effects. There is no specific antidote for sumatriptan overdose. Management consists of supportive care, including continuous ECG monitoring for at least 12 hours, and monitoring of vital signs. It is crucial to seek immediate emergency medical attention or contact a Poison Control center in the event of a suspected overdose.
Storage
Store Imitrex at room temperature between 20°C to 25°C (68°F to 77°F). Excursions are permitted between 15°C and 30°C (59°F and 86°F). The autoinjector and nasal spray devices should be protected from light. Keep all medications out of the reach of children and pets. Do not use after the expiration date printed on the packaging.
