Zovirax (acyclovir) is a pioneering antiviral medication specifically formulated to manage infections caused by the herpes simplex virus (HSV) and varicella-zoster virus (VZV). As a nucleoside analogue DNA polymerase inhibitor, it works by inhibiting viral replication, thereby reducing the severity and duration of outbreaks. This medication is available in multiple formulations, including oral tablets, topical cream, and intravenous solutions, to address a spectrum of clinical needs from episodic treatment to suppressive therapy. Trusted by healthcare professionals for decades, Zovirax remains a cornerstone in antiviral therapy, offering targeted action with a well-established safety profile.

Features

  • Active ingredient: Acyclovir
  • Available formulations: 200mg, 400mg, 800mg oral tablets; 5% topical cream; intravenous injection
  • Mechanism: Selective inhibition of viral DNA polymerase
  • Prescription-only medication in most regions
  • Manufactured under stringent quality control standards
  • Multiple packaging options for patient convenience

Benefits

  • Rapid reduction in viral shedding and lesion formation
  • Shortens healing time for primary and recurrent herpes outbreaks
  • Decreases the frequency of recurrent episodes with suppressive therapy
  • Helps prevent transmission of genital herpes to susceptible partners
  • Minimizes risk of complications in immunocompromised patients
  • Well-tolerated with extensive clinical experience supporting its efficacy

Common use

Zovirax is primarily indicated for the treatment of herpes simplex virus infections, including genital herpes, herpes labialis (cold sores), and herpes encephalitis. It is also used for the treatment of varicella (chickenpox) and herpes zoster (shingles) in both immunocompetent and immunocompromised patients. Additionally, it serves as prophylaxis against herpes infections in transplant recipients and patients undergoing chemotherapy.

Dosage and direction

Oral Tablets:

  • Genital herpes (initial episode): 200mg every 4 hours (5 times daily) for 10 days
  • Genital herpes (recurrent episodes): 200mg every 4 hours (5 times daily) for 5 days
  • Chronic suppressive therapy: 400mg twice daily
  • Herpes zoster: 800mg every 4 hours (5 times daily) for 7-10 days
  • Chickenpox: 20mg/kg (max 800mg) four times daily for 5 days

Topical Cream:

  • Apply to affected area every 4 hours (6 times daily) for 7 days

Intravenous:

  • 5-10mg/kg every 8 hours for 7-10 days (dose varies by indication and renal function)

Always complete the full course of treatment as prescribed. Dosage adjustments are necessary for patients with renal impairment.

Precautions

  • Maintain adequate hydration during treatment to prevent crystalluria
  • Use with caution in elderly patients and those with pre-existing renal conditions
  • Monitor renal function during intravenous therapy
  • Not a cure for herpes infections; does not eliminate the virus from the body
  • Does not prevent sexual transmission of herpes; barrier protection still recommended
  • Topical formulation should not be applied to mucous membranes or eyes

Contraindications

  • Hypersensitivity to acyclovir, valacyclovir, or any component of the formulation
  • Patients with history of severe neurological reactions to previous antiviral therapy
  • Concurrent administration with other nephrotoxic drugs in patients with renal impairment
  • Topical formulation contraindicated in patients with known hypersensitivity to propylene glycol

Possible side effect

Common side effects (≥1%):

  • Nausea (2-5%)
  • Headache (2-4%)
  • Diarrhea (1-3%)
  • Vomiting (1-2%)
  • Dizziness (1-2%)

Less common side effects:

  • Elevated liver enzymes
  • Rash
  • Fatigue
  • Abdominal pain

Rare but serious side effects:

  • Nephrotoxicity (especially with IV administration)
  • Neurotoxicity (agitation, confusion, hallucinations)
  • Thrombotic thrombocytopenic purpura/hemolytic uremic syndrome
  • Anaphylaxis

Drug interaction

  • Probenecid: Increases acyclovir plasma concentration
  • Other nephrotoxic drugs (aminoglycosides, cyclosporine): Increased risk of renal impairment
  • Zidovudine: May cause increased drowsiness and lethargy
  • Mycophenolate mofetil: Potential additive effect on immunosuppression
  • Monitor closely when administered with medications that compete for renal tubular secretion

Missed dose

Take the missed dose as soon as remembered, unless it is almost time for the next scheduled dose. Do not double the dose to make up for a missed one. Maintain the regular dosing schedule. For topical formulation, apply as soon as remembered and continue with regular application schedule.

Overdose

Symptoms may include agitation, coma, seizures, and renal impairment. Management includes supportive care and hemodialysis, which significantly enhances acyclovir clearance. Maintain adequate urinary output to prevent crystall formation. Contact poison control center immediately for guidance.

Storage

Store at room temperature (15-30°C/59-86°F). Keep in original container, tightly closed. Protect from moisture and light. Do not freeze. Keep all medications out of reach of children and pets.
