Common use

  3. Shift Work Sleep Disorder (SWSD): For individuals who experience insomnia during scheduled sleep times or excessive sleepiness during scheduled wake times due to a work schedule that overlaps with the typical sleep period.

Its off-label use, which should only be considered under strict medical supervision, sometimes includes the management of fatigue associated with conditions like multiple sclerosis or depression. It is also sometimes sought for cognitive enhancement, though this is not an approved indication.

Dosage and direction

The dosage of Waklert must be individualized based on the condition being treated and the patient’s response. It is typically administered as a single daily dose in the morning.

  • For Narcolepsy or OSA: The recommended dose is 150 mg or 250 mg taken orally once daily in the morning.
  • For Shift Work Sleep Disorder: The recommended dose is 150 mg taken orally approximately 1 hour prior to the start of the work shift.

The tablet should be swallowed whole with a glass of water. It can be taken with or without food, although a high-fat meal may delay the absorption and time to peak concentration. Dosage adjustments are necessary for patients with severe hepatic impairment, with a recommended dose of 50 mg. The safety and efficacy in pediatric patients have not been established.

Precautions

  • Cardiovascular Issues: Use with caution in patients with a history of left ventricular hypertrophy, mitral valve prolapse, or other significant cardiovascular problems, as increases in blood pressure and heart rate have been observed.
  • Psychiatric History: May cause psychiatric symptoms such as anxiety, agitation, or depression. Use with extreme caution in patients with a history of psychosis, mania, or depression.
  • Dependency Potential: Although the potential is low, Waklert is a controlled substance and has some potential for psychological or physical dependence. A history of drug or alcohol addiction is a risk factor.
  • Driving and Operating Machinery: Patients should be cautioned about operating automobiles or hazardous machinery until they know how the drug affects them, as it may cause dizziness or blurred vision.
  • Pregnancy and Lactation: Waklert is classified as Pregnancy Category C. There are no adequate and well-controlled studies in pregnant women. It should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known if Armodafinil is excreted in human milk; a decision should be made to discontinue nursing or discontinue the drug.

Contraindications

Waklert is contraindicated in patients with:

  • Known hypersensitivity to Armodafinil, Modafinil, or any of the inactive ingredients in the formulation.
  • A history of serious skin reactions, such as Stevens-Johnson Syndrome (SJS), Toxic Epidermal Necrolysis (TEN), or Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS), to Modafinil or Armodafinil.

Possible side effect

Like all medications, Waklert can cause side effects, although not everybody gets them. Common side effects (≥5%) include:

  • Headache
  • Nausea
  • Dizziness
  • Insomnia
  • Anxiety
  • Dry mouth
  • Diarrhea
  • Palpitations

Serious but less common side effects require immediate medical attention:

  • Serious Skin Rashes: Including SJS and TEN, which can be life-threatening. Discontinue at first sign of rash.
  • Angioedema and Anaphylactoid Reactions: Symptoms include swelling of the face, eyes, lips, tongue, or throat; trouble swallowing or breathing; and hives.
  • Multi-organ Hypersensitivity (DRESS): A systemic reaction presenting with fever, rash, and organ dysfunction (e.g., liver, kidney).
  • Persistent Sleepiness: If a patient experiences no improvement in, or an increase in, sleepiness, they should contact their physician.
  • Psychiatric Symptoms: Including depression, mania, hallucinations, suicidal ideation, and aggression.

Drug interaction

Waklert can interact with several other medications, primarily due to its effect on hepatic enzymes (it is a weak inducer of CYP3A4 and an inhibitor of CYP2C19).

  • Hormonal Contraceptives: Armodafinil may reduce the effectiveness of estrogen-based contraceptives (e.g., birth control pills, patches, rings). Women of childbearing potential should use an alternative or additional method of contraception while taking Waklert and for one month after discontinuation.
  • Cyclosporine: Levels may be reduced; monitoring of cyclosporine blood levels is recommended.
  • CYP2C19 Substrates: (e.g., Omeprazole, Diazepam, Phenytoin) Armodafinil may increase their plasma concentrations.
  • Warfarin: May alter the metabolism of warfarin; closer monitoring of prothrombin time/INR is advised.
  • Other CNS Active Drugs: May have additive effects when taken with other stimulants or drugs that affect neurotransmitters (e.g., MAOIs should not be co-administered).

Missed dose

If a dose is missed, it should be taken as soon as possible on the same day. However, if it is late in the day, the missed dose should be skipped to avoid nighttime insomnia. Do not double the next dose to make up for a missed one.

Overdose

Symptoms of overdose are likely to be an exaggeration of the known pharmacological effects, including:

  • Agitation or excitation
  • Insomnia
  • Anxiety
  • Tremor
  • Tachycardia (elevated heart rate)
  • Hypertension (elevated blood pressure)
  • Diarrhea
  • Hallucinations

In case of suspected overdose, symptomatic and supportive medical treatment should be initiated. There is no specific antidote for Armodafinil overdose. The patient should be monitored in a controlled environment, and hemodialysis is not expected to be effective due to high protein binding.

Storage

  • Store at room temperature, between 20°C to 25°C (68°F to 77°F).
  • Excursions are permitted between 15°C and 30°C (59°F and 86°F).
  • Keep the medication in its original container, tightly closed, and out of reach of children and pets.
  • Protect from excessive moisture, light, and heat.
  • Do not flush medications down the toilet or pour them into a drain unless instructed to do so. Properly discard this product when it is expired or no longer needed via a medicine take-back program.
