It is a first-line recommendation for pain management in scenarios where anti-inflammatory effects are not required or are contraindicated.

Dosage and direction

Dosage is dependent on age and the specific product formulation. Always read and follow the product labeling. Do not exceed the maximum daily dose.

  • Adults and children 12 years and older: The typical dose is 325 mg to 650 mg every 4 to 6 hours, or 1000 mg every 6 to 8 hours, as needed. Do not exceed 3000 mg in 24 hours unless directed by a doctor; many experts now recommend a maximum of 3000 mg to enhance safety. Some labels advise a maximum of 4000 mg. Precautions

  • Liver Warning: This product contains acetaminophen. Severe liver damage may occur if you take: more than the maximum daily dose, with other drugs containing acetaminophen, or 3 or more alcoholic drinks every day while using this product.
  • Do not use with other medicines containing acetaminophen. Prescription and OTC drugs may contain acetaminophen; it is the patient’s responsibility to check labels.
  • Ask a doctor before use if you have liver disease or a history of alcoholism.
  • Stop use and ask a doctor if pain worsens or lasts more than 10 days, fever worsens or lasts more than 3 days, new symptoms occur, or if redness or swelling is present, as these could be signs of a serious condition.
  • Keep out of reach of children. In case of overdose, get medical help or contact a Poison Control Center right away. Prompt medical attention is critical for potential acetaminophen overdose, even if initial symptoms are absent.

Contraindications

Tylenol is contraindicated in patients with:

  • Known severe hypersensitivity to acetaminophen or any component of the formulation.
  • Severe active liver disease or severe hepatic impairment.

Possible side effect

Although rare at recommended doses, the following side effects may occur. In the event of any allergic reaction, discontinue use immediately.

  • Allergic reactions: skin rash, hives, itching, swelling of the face, throat, tongue, or lips.
  • Serious skin reactions: Very rare but potentially fatal skin reactions (e.g., Stevens-Johnson Syndrome, toxic epidermal necrolysis) can occur. Stop use and seek medical attention immediately if a skin rash develops.
  • At high doses or in cases of overdose: nausea, vomiting, loss of appetite, sweating, extreme tiredness, unusual bleeding or bruising, pain in the upper right stomach, and yellowing of the skin or eyes (jaundice), which are signs of liver toxicity.

Drug interaction

Acetaminophen can interact with several medications, potentially altering their effects or increasing the risk of liver damage.

  • Warfarin (Coumadin, Jantoven): Long-term use of acetaminophen may increase the anticoagulant effect of warphen, increasing the risk of bleeding. INR should be monitored closely.
  • Isoniazid: May increase the risk of acetaminophen-induced hepatotoxicity.
  • Other Acetaminophen-containing products: Significantly increases the risk of overdose and acute liver failure.
  • Drugs that induce liver enzymes: Such as anticonvulsants (e.g., carbamazepine, phenytoin, phenobarbital) and the anti-tuberculosis drug rifampin, may increase the metabolism of acetaminophen to its toxic metabolite, potentially increasing hepatotoxicity risk, even at therapeutic doses.
  • Alcohol: Chronic, heavy alcohol use increases the risk of severe liver injury from acetaminophen. Consumers should avoid or limit alcohol intake while taking this medication.

Missed dose

Tylenol is taken on an as-needed basis for pain or fever. It is not intended for scheduled, around-the-clock dosing. If you are using it regularly and miss a dose, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose and resume your usual dosing schedule. Do not double the dose to catch up.

Overdose

Acetaminophen overdose is a medical emergency that can be fatal. Early symptoms of overdose may include nausea, vomiting, sweating, and general malaise. These initial symptoms may subside, leading to a false sense of recovery. Within 24 to 72 hours, clinical signs of liver toxicity may appear (pain in the upper right abdomen, jaundice, coagulopathy). Immediate medical attention is critical, even if symptoms are not present. Do not wait for symptoms to appear. Contact a poison control center (1-800-222-1222) or emergency services immediately. The antidote, N-acetylcysteine (NAC), is most effective when administered within 8-10 hours of ingestion but may still be beneficial when given later.

Storage

  • Store at room temperature (20°-25°C or 68°-77°F).
  • Keep the bottle tightly closed and protect from excessive moisture and heat.
  • Do not store in the bathroom.
  • Keep all medications out of sight and reach of children and pets to prevent accidental ingestion.
