Cialis (tadalafil) is a prescription phosphodiesterase type 5 (PDE5) inhibitor clinically proven to treat erectile dysfunction (ED) and benign prostatic hyperplasia (BPH). It functions by enhancing blood flow to the penile tissues, facilitating the achievement and maintenance of an erection sufficient for sexual activity. Unlike shorter-acting alternatives, its extended half-life offers a significantly prolonged therapeutic window. This medication represents a cornerstone in urological and sexual medicine, providing a reliable option for men seeking to reclaim sexual health and performance.

Features

  • Active ingredient: Tadalafil
  • Available in 2.5 mg, 5 mg, 10 mg, and 20 mg oral tablets
  • Onset of action typically within 30 minutes to 2 hours
  • Duration of efficacy up to 36 hours
  • Approved for both erectile dysfunction and benign prostatic hyperplasia
  • Can be taken with or without food (high-fat meals may delay absorption)

Benefits

  • Enables reliable and sustained erectile response for up to 36 hours, reducing time pressure and supporting spontaneity.
  • Improves urinary flow and reduces symptoms associated with benign prostatic hyperplasia, such as urgency and frequency.
  • Enhances overall sexual confidence and relationship satisfaction by providing predictable performance.
  • Offers flexible dosing options, including daily low-dose regimens for continuous readiness.
  • Supported by extensive clinical research and long-term safety data in diverse patient populations.
  • Does not require sexual stimulation to work but is effective only when combined with it.

Common use

Cialis is primarily prescribed for the treatment of erectile dysfunction (ED) in adult men. It is also indicated for the signs and symptoms of benign prostatic hyperplasia (BPH), either alone or in combination with finasteride. In some cases, it may be used off-label for pulmonary arterial hypertension under strict medical supervision. It is intended for use only by patients with a confirmed diagnosis and prescription from a qualified healthcare provider.

Dosage and direction

For erectile dysfunction (as-needed dosing): The recommended starting dose is 10 mg taken prior to anticipated sexual activity. Depending on efficacy and tolerability, the dose may be increased to 20 mg or decreased to 5 mg. The maximum recommended dosing frequency is once per day.

For daily use: A dose of 2.5 mg or 5 mg is taken once daily, approximately at the same time each day, without regard to timing of sexual activity.

For benign prostatic hyperplasia: 5 mg taken once daily at approximately the same time each day.

Tablets should be swallowed whole with a glass of water; do not crush or split. Sexual stimulation is required for an erection to occur.

Precautions

  • Not intended for use by women, children, or adolescents.
  • Use with caution in patients with cardiovascular disease; sexual activity poses a potential cardiac risk.
  • Avoid consumption of grapefruit or grapefruit juice, as it may increase tadalafil concentrations.
  • May cause hypotension; caution advised in patients on antihypertensive therapy.
  • Priapism (prolonged erection >4 hours) is a rare but serious condition requiring immediate medical attention to avoid permanent damage.
  • Use in patients with severe renal or hepatic impairment is not recommended or requires dose adjustment.
  • May cause non-arteritic anterior ischemic optic neuropathy (NAION)—discontinue use if sudden vision loss occurs.

Contraindications

  • Concomitant use of organic nitrates (e.g., nitroglycerin, isosorbide mononitrate) or nitric oxide donors.
  • History of hypersensitivity to tadalafil or any component of the formulation.
  • Patients with severe cardiovascular instability, including recent myocardial infarction, unstable angina, or uncontrolled arrhythmias.
  • Use in patients with retinitis pigmentosa or other hereditary retinal disorders (unless benefits outweigh risks).

Possible side effect

Common side effects may include:

  • Headache
  • Dyspepsia
  • Back pain
  • Myalgia
  • Nasal congestion
  • Flushing
  • Dizziness

Less common but serious side effects:

  • Sudden hearing loss or tinnitus
  • Vision changes or loss
  • Priapism
  • Chest pain or arrhythmia
  • Severe hypotension

Drug interaction

  • Nitrates: Concomitant use is contraindicated due to risk of severe hypotension.
  • Alpha-blockers: May potentiate hypotensive effects; dose adjustment or separation may be required.
  • CYP3A4 inhibitors (e.g., ketoconazole, ritonavir): May increase tadalafil exposure; dose reduction recommended.
  • Antihypertensives: Additive blood pressure-lowering effects possible.
  • Alcohol: Excessive consumption may increase risk of hypotension, dizziness, or increased heart rate.

Missed dose

For as-needed dosing: Take as soon as remembered if sexual activity is still anticipated. Do not double the dose.

For daily dosing: Take the missed dose as soon as remembered, unless it is almost time for the next dose. In that case, skip the missed dose and resume the regular schedule. Do not take two doses within 24 hours.

Overdose

Symptoms may include severe headache, flushing, dizziness, nausea, vomiting, fainting, or prolonged erection. In case of suspected overdose, seek immediate medical attention. Supportive measures should be implemented, including cardiovascular monitoring. Hemodialysis is not expected to significantly enhance elimination.

Storage

Store at room temperature (15–30°C or 59–86°F) in the original container. Protect from light and moisture. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging.
