Careprost is a topical ophthalmic solution containing the active pharmaceutical ingredient Bimatoprost, a synthetic prostaglandin analog. It is the first and only FDA-approved prescription treatment for hypotrichosis of the eyelashes, a condition characterized by inadequate or not enough eyelashes. Its primary mechanism of action involves extending the anagen (growth) phase of the eyelash hair cycle, increasing the percentage of hairs in this active phase, and stimulating the development of new, thicker, and darker lashes. This prescription-grade treatment offers a scientifically validated, non-invasive alternative to cosmetic enhancers for individuals seeking significant and measurable improvements in lash prominence, length, and overall aesthetic appeal.

Features

  • Active Ingredient: Bimatoprost 0.03% (0.3 mg/mL)
  • Formulation: Sterile, isotonic, buffered ophthalmic solution
  • Presentation: Supplied in a multi-dose 3 mL plastic bottle with a sealed, sterile applicator brush
  • Prescription Status: Rx-only medication
  • Mechanism: Prostaglandin analog/prostamide F2α receptor agonist

Benefits

  • Achieves a measurable and significant increase in overall eyelash prominence, including length, thickness, and darkness.
  • Transforms the eyelash hair cycle by prolonging the active growth (anagen) phase and increasing the number of lashes in this phase.
  • Provides a medically approved, evidence-based solution for inadequate eyelashes (hypotrichosis).
  • Delivers gradual, natural-looking results that are sustainable with continued use.
  • Offers a targeted, topical application that minimizes systemic exposure when used as directed.

Common use

Careprost is indicated for the treatment of hypotrichosis of the eyelashes. It is prescribed to adult patients (18 years and over, including the elderly) who desire to enhance eyelash growth in terms of length, thickness, and darkness. Its use is considered both therapeutic for the diagnosed condition and aesthetic for the resulting improvement in appearance.

Dosage and direction

The recommended dosage is one application per day, in the evening, to the skin of the upper eyelid margin at the base of the eyelashes. Using the sterile, single-use applicator supplied, apply one drop of the solution to the applicator. Carefully draw the applicator across the skin of the upper eyelid margin at the base of the eyelashes, going from the inner to the outer part of the lid. Do not apply to the lower lid. Any excess solution outside the upper lid margin should be blotted away with a tissue. To avoid contamination, discard each applicator after a single use. Do not allow the tip of the bottle to contact surrounding structures, fingers, or any other surface. Contact lenses must be removed prior to application and may be reinserted 15 minutes afterward.

Precautions

  • Strict Aseptic Technique: To minimize the risk of eye infections, avoid contaminating the applicator or bottle tip.
  • Potential for Iris Pigmentation: Bimatoprost may gradually increase the brown pigmentation of the iris. This change is likely to be permanent and may result in heterochromia (eyes of different colors) in patients with mixed-color irides.
  • Potential for Eyelid Skin Darkening: Reversible darkening of the eyelid skin may occur.
  • Potential for Hair Growth: Hair growth may occur in skin areas that the solution frequently contacts.
  • Intraocular Pressure (IOP) Monitoring: While used for eyelash growth, Careprost can lower IOP. Patients using it concomitantly with IOP-lowering prostaglandin analogs for glaucoma should be monitored.
  • Special Populations: Use during pregnancy or breastfeeding is not recommended unless the potential benefit justifies the potential risk to the fetus or infant.

Contraindications

Careprost is contraindicated in patients with:

  • Known hypersensitivity to Bimatoprost or any of the other ingredients in the formulation (benzalkonium chloride, sodium chloride, sodium phosphate dibasic, citric acid, and purified water).
  • Active or suspected ocular or periocular infections (e.g., conjunctivitis, keratitis, blepharitis).

Possible side effect

The most common ocular side effects, typically mild to moderate, include:

  • Pruritus (itching of the eyes)
  • Conjunctival hyperemia (eye redness)
  • Skin hyperpigmentation (darkening of the eyelid skin)
  • Ocular dryness
  • Iris pigmentation changes (increased brown pigmentation)
  • Eye irritation
  • Foreign body sensation
  • Growth of eyelashes in unintended areas (e.g., adjacent skin)
  • Less common but more serious side effects can include intraocular inflammation (uveitis, iritis), macular edema (particularly in aphakic patients, pseudophakic patients with a torn posterior lens capsule, or patients with known risk factors for macular edema), and eyelid reactions such as edema (swelling).

Drug interaction

Formal drug interaction studies have not been conducted with Careprost. However, due to its pharmacological class, there is a potential for an additive effect when used with other IOP-lowering prostaglandin analogs (e.g., Latanoprost, Travoprost). Concomitant use of multiple prostaglandin analogs is generally not recommended. The preservative benzalkonium chloride may be absorbed by soft contact lenses.

Missed dose

If a dose is missed, the patient should apply the solution at the next scheduled evening application. The patient should not apply a double dose to make up for the missed one, as this increases the risk of side effects without enhancing efficacy.

Overdose

Ocular overdose is not expected to produce life-threatening systemic effects. In case of accidental ingestion, symptomatic and supportive care is recommended. Ocular overdose may be flushed from the eye(s) with lukewarm tap water. Medical attention should be sought if symptoms are severe or persistent.

Storage

Store the unopened bottle in a refrigerator at 2°C to 8°C (36°F to 46°F). Once opened, the bottle may be stored at room temperature up to 25°C (77°F) for 6 weeks. Do not freeze. Keep the bottle tightly closed in its original carton to protect from light. Keep out of the reach and sight of children.

Reviews

  • Clinical Trial Data: In pivotal multicenter, double-masked, randomized, vehicle-controlled studies, patients using Bimatoprost 0.03% for 4 months demonstrated statistically significant improvements from baseline in eyelash growth across all primary efficacy endpoints (global eyelash assessment, eyelash length, thickness, and darkness) compared to vehicle. Patient satisfaction scores were significantly higher in the active treatment group.
  • Dermatologist, New York: “In my practice, Careprost remains the gold-standard, prescription-only treatment for true hypotrichosis. The results are quantifiable and predictable for suitable patients. My role is to properly screen candidates, manage expectations regarding the timeline for results and potential side effects like periocular pigmentation, and ensure safe application technique.”
  • Ophthalmologist, London: “While its primary use in my clinic is for glaucoma, the eyelash growth effect is undeniable. When prescribing it cosmetically, I conduct a thorough baseline ocular exam, including iris documentation, and counsel patients extensively on the risk of permanent iris color change. It is a potent medication, not a cosmetic, and must be treated as such.”
  • Patient, 42: “After being diagnosed with hypotrichosis following chemotherapy, my dermatologist prescribed Careprost. The results were not instant, but after 12 weeks, the difference was remarkable. My lashes are longer and fuller than they were even before treatment. The application is a precise routine, but well worth it for the confidence it has restored.”
